Principals of Enablement
Team Motivate is fully committed to enabling people to live a life of their own choosing, in their own home, for the whole of their life, as included members of their community. To develop and provide Supported Living Services for adults with special needs of both gender, age 18 and over and who may also have learning difficulties or autistic spectrum disorder.

Team Motivate works alongside people according to their needs and abilities, for as long as necessary, often over extended periods, to build their capacities, positive experiences, and successful outcomes. Team Motivate focus is on improving service users quality of life, enabling them to realise their goals, aspirations and wishes.
Full time / Part time
Team Motivate provides full-time/part-time Supported Living Service and high-quality accommodation through housing associations if required within the community, for vulnerable people and people with mild to complex needs. This service is designed to support the individual wants and needs in the most effective way. We ensure the individual is involved in creating their support plan, so we can tailor make the support plan to the individual as we know everyone is unique.

The service is staffed 24 hrs and support is offered around, housing and tenancy management, daily living skills, social inclusion, leisure activities, enablement, and integration into the community. Service users in our Supported Living programme experience greater choice, support from a smaller team which enables greater consistency and continuity of support and the provision of a physical environment that the service user can truly call their own.

​Team Motive recognise that locating the right support can be challenging, especially when sustaining your/their independence. We design our Supported Living around the wants and needs of the individual. Team Motivate provides an alternative to residential care as we empower individuals by letting you choose the amount of support and when you need it, so you can live a fulfilled life.
Planning your
Supported Living
When planning your Supported Living, you will be able to personalise your plan based around your interest, priorities, and skills with our experienced staff.

This can entail helping with personal care needs, managing finance, finding education, work, or recreational activities.
Different Support Types
You can choose to live independently in your own property and tell us how much support you will need. We can work together to find a flat that will meet your needs and wants.

We can connect you with other people that want to live independently but are not ready to live alone or prefers company.
SHARED
LIVING
This type of support will enable you to plan your future living in a structured way. We will equip you with the right skills needed to live independently and when you are ready, we will support your move.
STEPPING

SUPPORT
We encourage you to build relationships with key workers and follow a defined plan on the road to a sustainable outcome. Families can play a key role in the process by spending time at the project and engaging with the creation of a person-centred support plan with staff members.
​Working closely with you, your family and external professionals from the referral stage onwards helps us build strong relationships from the outset that, we believe, leads to better outcomes.
Service users will be able to exercise their influence over the day to day running of their home and to make choices in all aspects of their daily lives.
Service users are supported using PBS and we can offer various communication tools including Makaton and PES.

Independent Living Transition training for those who need support to recover their lives.

At night there will be a minimum of one waking night staff with the full support of an on-call system.
​
A key worker system allowing better awareness of each individual’s social, emotional, physical, cultural, sexual and spiritual needs
They have opportunities for volunteering and community outreach.

A comprehensive assessment is completed prior to and during the first month of residence.
​
Service users are supported in maintaining their existing social networks and in developing new networks.
​
The level of support offered is individually tailored to each service users need.

This forms the basis of the plan of care and risk management strategies.

Giving the service user continuity and supporting them to deliver their personal care plan.

We can cover shared support or one-to-one support depending on the service users’ individual needs.

Thereafter, regular in-House reviews occur with the service user to ensure that these remain congruent with the individual’s needs.
Team Motivate recognises and values cultural diversity. Service users are able to experience an environment in which their cultural and/or sexual identity is supported and strengthened.
Long Term Support
Team Motivate offers high quality Supported Living services and accommodation for people who require long term support with a variety of diagnosis, including learning disabilities, autism, mental health, physical disabilities, acquired brain injuries and behaviours that are deemed to be challenging.
